Our sick visit walk in clinic is for patients who are ill, or have urgent concerns. While long standing concerns, educational and mental health concerns are important, we CANNOT deal with them in the rapid pace of this time. We will do our best to keep wait times down, but we ask for your patience.

There is a $25 charge for online/phone prescription renewals. Please include the name and dose of the medication, as well as the pharmacy address and FAX number.

We typically charge $25-$50 for most forms, some more depending on the time it takes to fill. We usually require 1-2 weeks to complete most forms. Forms that require a PHYSICAL EXAM require an appointment.

2023 Flu Shot Clinics at Clairhurst are done.
We have very limited appointments for flu shots for 6mths – 6yrs.
Over 6yrs should go to their local pharmacy.
If you have a regular check up (any age) in November/December/January – ask for a flu shot – if we have supply we will do it!
