Some of the frequently asked questions we receive:

There are a number of options in the neighbourhood.

Green P Parking on the south east corner of Bathurst & St Clair.

Green P Parking under the building directly south of us, entrance is on Vaughan Road.

Green P Parking behind Tim Hortons, entrance on Kenwood Avenue, west on St Clair from Bathurst, on the north side.

Meter Parking on the street on the south side of St Clair west of Bathurst and on the east side of Vaughan Road south of St Clair.

Phones are answered from 9.00am – 4pm Mon – Thurs and 9.00am – 2.30pm Friday.

Clairhurst patients should use our walk in. Traffic and parking can be busy, please allow extra time as walk in closes sharp.
On weekends and holidays, the on call Dr will be posted on our website.
If this is an emergency, go directly to your nearest Hospital or Emergency Department

Call your pharmacy, they may have repeats still available, if not, ask them to fax a repeat request to us. 416-531-1639 or use the prescription form on our website.

Should the doctor need to see the patient before refilling we will contact you.
There is a charge for a prescription refill

Use the form on this website to request your immunization/vaccine record.
Fill out one form per child and we will provide your child’s vaccine record

Forms take a couple of weeks, be sure to fill out the patient name and demographics as well as your part of the form before sending it to us via the form requirements tab on our website.


If you would like the form emailed back to you please let us know and we will email it to the email address on the patient file unless you tell us otherwise. We cannot email forms to a third party; parents and legal guardians only.


Check the form and if it requires a recent physical or blood pressure you need to book an appointment with your doctor (many camp and school trip forms require a visit to your doctor). Please plan ahead as we cannot accommodate last minute appointments for form check-ups we book a few months in advance.


Forms must be filled out by your own doctor. If your doctor is out of the office you will need to wait for their return so planning ahead is absolutely necessary to avoid disappointment.


Forms are not covered by OHIP, there will be a charge.

If you need a referral to a specialist, you will need your pediatrican or GP to fax a referral to the specialists office.  You will need to see your doctor for the issue before a referral is provided.

Annual check ups are booking 4months in advance.  We have limited space for follow-up or non sick issues.

Of course if your child is unwell or the condition cannot wait Clairhurst patients can come to our walk in.

We do not have sick child appointments.

We understand its frustrating as our phone lines are constantly busy, use our website to send a message and we’ll get back to you.  We respond daily to most website messages.

We endeavor to return phone calls and messages in a timely manner,

However, with phone messages, often bad reception from mobile phones, inaudible (speaking quietly, background noise, not leaving a clear phone number or name) prevents us from returning calls.

Website messages: This method is much easier as we have the patients details so we can look up the details and phone numbers that are often typed in in-correctly.

Please bear in mind that requests often need an answer or action from the doctor.  Our doctors are busy with patients throughout the day, so often it takes a few days to compile the information needed to respond.  Sometimes your pediatrician is on vacation and your message waiting for them to return.

Not all services are covered by OHIP.  You will receive an invoice if you have received a service that your OHIP does not cover.  For example: letters, camp forms, prescription repeats and physicals, missed appointments or expired OHIP cards.

OHIP Card – Please bring the patients valid health card to every visit. If the card is invalid (example: expired, reported cancelled or lost) you will be asked to pay for the visit.

If your baby is a newborn, the OHIP number from the hospital is only temporary. You must register with OHIP as soon as possible to receive a permanent OHIP card.

If your child does not have OHIP but you have private insurance coverage you will be billed for the appointment, a receipt can be provided for you to submit to claim it back from your insurance.

Communication:  Please us the website to send a message, most messages  are returned within 48hrs. Some messages require longer to get the answers for you.  You may also call but our phone lines are busy during business hours, it may be more timely to use the website.

Please be advised that Clairhurst Pediatrics has a zero-tolerance policy regarding abusive or violent behaviour. Any mistreatment of our admin staff, nurses or doctors whether verbal or written, will be taken seriously and may result in termination of services at our clinic.

 Appointments: Appointments for well checks should be made via online booking well in advance.  If you have trouble with our online booking system please contact us via the website.

We require 48hrs business days notice to cancel/reschedule an appointment. Last minute cancellations are subject to a fee of $75 for general pediatricians and $100- $200 for a specialist.  This charge also applies if the patient does not show up for their appointment.

Arrive on time: If you are more than 10 minutes late for your appointment it may need to be rescheduled to a later date.  We will not hold up other patients appointments to squeeze you in.  If you missed your appointment time you have missed the appointment. Traffic and parking can be busy so leave plenty of extra time to arrive on time.

Allergies: Please refrain from bringing food into the office to help us protect our patients with severe food allergies.

Parental Custody: Clairhurst policy regarding communication and record access to separated parents.  Legal documentation must be provided for the patients file for proof of custody.  Please read the following guidelines:

Parents with Sole Custody: Only the parent with sole custody will have access to the child’s medical records and ability to book/cancel and attend appointments. The chart will only contain the sole custodial parent’s contact information.

Parents with joint custody:  Legal Documentation: We require parents to submit the current legal custody agreement from the court.  Once this documentation is in the patients file we will update the patient file with contact information for both parents. Access to care: Both parents have access to the child’s medical care and booking appointments.   Information sharing:  Both parents should join the same appointment to receive updates together.  Our staff (admin/doctor/nurse) are not responsible for sharing information to both parents after a visit.  It is the parents responsibility to either attend the appointment together  or communicate with each other to stay up to date with their child’s medical care.  It is the parents responsibility to share the information between themselves.  Communication by email:  Admin communication is often conducted by Ocean email.  This system only allows for one email address to be entered per patient.  The email address we would use is the one provided during the initial registration.  We will not change this email address at the request of one parent. Both parents must consent and provide the email address if it needs to change. It is strongly recommended that the parents create an email address that they both have access to and then both provide it to us as the primary email.  Confidential Information:  When our provider shares confidential information or want to reach the parents via email, they will use the primary email on file or phone number on file. It is the primary contact holder’s responsibility to update the other parent.  We are not responsible for emailing, contacting the other parent with the same information.  Medical Record Access:  Medical records and visit notes are considered internal provider records and are maintained for the continuity of care. These records are only shared with other medical providers as part of transfer of care or for a medical opinion and only with the consent of both parents.  They are not intended for routine family updates; if you are looking for these records to update because you did not attend your child’s appointment, please remember that both parents are to communicate with each other to stay updated.  Please coordinate with the other parent for more details of appointments as needed, we will not be responsible for sharing these records. We remind you that we recommend both parents attend the appointments and communicate with each other to stay updated.  If you need medical records for legal purposes, please have your lawyer request the medical record from us and we process this request.

Personal Information:  Clairhurst is committed to the confidentiality and security of personal information we collect.  We collect personal health information about you and your child(ren).  The information collected may be your name, date of birth, address, personal and family health information.  This information may be used by regulated and allied healthcare professionals to deliver patient care, administrative healthcare services and other services permitted by the law.  Your personal health information will only be shared with other physicians, specialists and healthcare professionals outside our clinic who are directly involved in your circle of care.
